In December of 1996, the Southern New York Association (SNYA) brought together a group of 25 New York State licensed voluntary and proprietary skilled nursing facility (SNF) operators (Founding Members) in order to form an SNF alliance to function as a single managed care contracting entity. The Founding members represented 34 SNF’s in the New York Metropolitan Area. The alliance was formed on December 9, 1996 as a New York limited liability company.

Founding members were committed to expanding the offering of services in communities they serve. The growth of managed Medicare and the coordination of services for patients requiring sub-acute care created a need for relationship building with the various health plans in the New York Area. Founding members proceeded to enter into a process of strategic planning to successfully market to managed care organizations. NYHCA’s strategic planning and vision foresaw group contracting as the conduit to expanding services offered in NYHCA facilities.

The mission and vision at the direction of the board was to engage in group contracts offering health plans and their members’ access to a large network of rehabilitation and sub-acute facilities. NYHCA was successful in pursuing these relationships and is currently contracted with all of the major health plans in the New York area offering sub-acute and rehabilitation services to Medicare, Medicaid and commercial populations. NYHCA also has relationships with the hospital systems and physicians groups to maximize the continuity of care offered to patient and resident populations.

NYHCA in its efforts to offer member facilities opportunities to maximize cost efficiency and provide additional services, is involved in a number of other business ventures related to group purchasing, rehabilitation services and long term care managed care plans. The New York Health Care Alliance currently represents a network of 48 sub-acute skilled nursing facilities located in the New York Metropolitan Area with facilities in the five boroughs, Nassau, Suffolk and Westchester.
